To request a medical exemption, go to Form Upload.
Download the Medical Exemption Request form and take it to your medical provider. Once they have completed the form, upload it back in the same place.
You will be asked to confirm that your upload looks correct. You will need to click on “Save” at the bottom of the page to complete the upload.
Your document will be under review (6). Our staff will reach out when they have either sent it for final approval. If there are any issues with your form, they will also communicate that.
After approval, the download will update, and your clearances will show as compliant.
