UC SHIP Waiver Application Instructions

The UC SHIP waiver is now closed.


For the 2012-2013 academic year, the University of California has adopted uniform UC SHIP waiver guidelines. Please note that these new guidelines are different than what UC Davis has had in the past. Your private insurance must meet specific criteria to waive UC SHIP.


Please take a moment to review and fill out the UC SHIP Waiver Criteria Worksheet so that you will be prepared to answer the new questions on the UC SHIP waiver application.


Waiver Deadlines

Be sure to submit your waiver application or payment IN ADVANCE of the fee payment deadline to avoid being dropped from your classes for non-payment. Fee payment deadlines are available through the UC Davis Office of the Registrar.

Waiver applications must be received by these dates in order to apply to the quarters indicated (NO EXCEPTIONS):

The University of California Student Health Insurance Plan (UC SHIP) is designed specifically for UC students with both Davis area and worldwide coverage.

UC SHIP combines medical, mental health and vision services offered by Student Health and Counseling Services at the Student Health and Wellness Center and at North Hall, including dental benefits for services that are not available on campus. Together, the medical, mental health, dental and vision insurance form UC SHIP.

For some student groups, UC SHIP waiver application deadlines are different from those listed above, based on the dates that their academic programs begin. Examples of this are LLM, MPVM, and FNP/PA students. Check the information for your student group on the new students page if your academic program begins on a different date than the beginning dates of coverage listed above.

You will be required to log into the waiver application using your UC Davis Login ID, your Kerberos Password and your Student ID#. Once you have logged into the UC SHIP waiver application, you will be re-directed to the Wells Fargo Insurance Services website where the waiver application is being processed.

Once you have completed your UC SHIP waiver application, you will be immediately notified if your waiver application was approved or denied and you will be sent an email confirmation of the decision.

Please remember that it is the responsibility of the student to prove that a waiver application has been submitted and approved. It is recommended that you print a copy of your approval for your records.

If your UC SHIP waiver application is denied, you will be provided a link to the UC SHIP Waiver Application Appeal form to fill out. This form must be filled out within two weeks of your waiver denial. Learn more about the appeal of waiver denial process.

If your UC SHIP waiver application is denied, you will be responsible to pay the UC SHIP fee posted to your student account. Failure to pay this fee may result in you being dropped from your classes. Find out more about the Office of the Registrar’s Dropped for Non-Payment policy.

Medical Students and Dual Degree Medical Students

Waivers for Medical Students and Dual Degree Medical Students are handled by the School of Medicine. This applies to dual degree students even while enrolled in the non-MD portion of your program. For more information, contact the School of Medicine Registrar’s Office at (916) 734-2409.
