Davis SHIP Waiver Application Instructions

The Davis SHIP Waiver deadline has been extended.

If you missed the waiver deadline of September 15th, you can still file a waiver application and have it considered for Fall quarter. Please download the Paper Davis SHIP Waiver and turn it in to the Insurance Services office by October 3rd. If it is received after October 3rd, it will only be processed for Winter and Spring quarters.

All registered UC Davis students are automatically enrolled in the campus sponsored UC Davis Student Health Insurance Plan (Davis SHIP).

If you have private insurance coverage and do not wish to be enrolled in Davis SHIP for the 2014-15 academic year, you will need to submit a waiver application by the stated deadlines. Failure to submit a waiver application by the deadline will result in you remaining enrolled in Davis SHIP and being responsible for the charges posted to your student account.


Waiver Deadlines

Approved waiver applications may take up to 10 business days to post to your student account if fees have already been assessed. If you submit your waiver application within 10 business days of the Fee Payment deadline, it is recommended that you pay all fees, including the Davis SHIP fee, on your student account. If your waiver is approved, your student account will be refunded the full Davis SHIP fee once your waiver posts to your student account.

Failure to pay all fees posted to your student account may result in you being dropped from your classes for non-payment. Fee payment deadlines are available through the UC Davis Office of the Registrar.

Waiver applications must be received by these dates in order to apply to the quarters indicated:

  • Deadline for Fall, Winter, & Spring Quarters
    Sept. 15, 2014
  • Deadline for Winter & Spring Quarters only Quarters
    Dec. 15, 2014
  • Deadline for Spring Quarter only
    Mar. 15, 2015
  • Deadline for Fall & Spring Semesters
    Aug. 15, 2014
  • Deadline for Spring Semester only
    Dec. 15, 2014

For some student groups, Davis SHIP waiver application deadlines are different from those listed above, based on the dates that their academic programs begin. Examples of this are LLM, MPVM, and FNP/PA students. Check the information for your student group on the new students page if your academic program begins on a different date than either August 11, 2014 (Semester) or September 29, 2014 (Quarter).

You will be required to log into the waiver application using your UC Davis Login ID, your Kerberos Password and your Student ID#. Once you have logged into the Davis SHIP waiver application, you will be re-directed to the Wells Fargo Insurance Services website where the waiver application is being processed.

Once you have completed your Davis SHIP waiver application, you will be immediately notified if your waiver application was approved or denied and you will be sent an email confirmation of the decision to your UC Davis email account.

Please remember that it is the responsibility of the student to prove that a waiver application has been submitted and approved. It is recommended that you print a copy of your approval for your records.

If your Davis SHIP waiver application is denied, you will be provided a link to the Davis SHIP Waiver Application Appeal form to fill out. This form must be filled out and submitted within 15 business days of your waiver denial. Learn more about the appeal of waiver denial process.

If your Davis SHIP waiver application is denied, you will be responsible to pay the Davis SHIP fee posted to your student account. Failure to pay this fee may result in you being dropped from your classes. Find out more about the Office of the Registrar’s Dropped for Non-Payment policy.

Medical Students and Dual Degree Medical Students

Waivers for Medical Students and Dual Degree Medical Students are handled by the School of Medicine. This applies to dual degree students even while enrolled in the non-MD portion of your program. For more information, contact the School of Medicine Registrar’s Office.
