UC SHIP Identification Cards                         

UC SHIP Dental

An ID card is not required to obtain dental services. However, if you want a UC SHIP Dental Plan ID card, you may obtain one from the Delta Dental of California "online services" webpage. To find a dentist and access dental services, follow the instructions in the "How to Use Dental Services" section of the UC SHIP Benefit Highlights (pdf). Your dentist will be able to verify your eligibility with the information you provide them.

UC SHIP Vision

A referral is not required to obtain vision services. Present your UC SHIP medical identification card for verification of benefits. Vision providers may call 1 (866) 940-8306 to verify benefits and eligibility.

UC SHIP Medical

UC SHIP medical plan identification cards are mailed directly to ALL UC SHIP members by Anthem Blue Cross each Fall in late October. Additionally, Anthem Blue Cross mails UC SHIP identification cards to NEW members throughout the year, usually within 6 weeks of the day their coverage begins.

If you have not received a UC SHIP ID card from Anthem Blue Cross, the instructions below describe how to verify or change your mailing address in SISWeb and how to obtain a temporary or permanent ID card.


Make Sure your Address is Correct

If you have moved but did not update your mailing address, your UC SHIP ID card may have been sent to your previous address. Any mailings you receive from Anthem Blue Cross, including your UC SHIP ID card, will be sent to the "Mailing/Current Address" that you currently have on file in your Health-e-Messaging profile. To verify or change your address, log in to Health-e-Messaging, click on "profile" and update your address and phone number.

Once you have verified or changed your address, read the following instructions to request a temporary and/or permanent ID Card.

Temporary ID Card

If you have not received a UC SHIP ID card, you may obtain a temporary card from the Member Access page of the Anthem Blue Cross website. You will be required to register as a user on the Anthem Blue Cross website, and will be asked for your "Member ID" number. Anthem Blue Cross provides a handout titled "Got ID Cards?" (pdf) that might be useful. If you do not know your insurance identification number, please contact the Insurance Services office at SHCS and we can provide it for you. You can also stop by the office and we can print a temporary ID Card for you.

Permanent ID Card

To obtain a permanent UC SHIP ID card, first update your mailing address in Health-e-Messaging. Allow two weeks for your updated address to be transferred to the Anthem Blue Cross customer database. Then log on to the Member Access page of the Anthem Blue Cross website and request a permanent ID card. It will be mailed to you directly from Anthem Blue Cross. If you do not know your insurance identification number, please contact the Insurance Services office at SHCS and we can provide it for you. You can also stop by the office and we can print a temporary ID Card for you.
