UC Student Health Insurance Plan
UC SHIP Eligibility Notice: Withdrawal or Cancellation of Registration

If you file a completed Notice of Cancellation/Withdrawal form with the Office of the Registrar BEFORE the quarter begins, your University of California Student Health Insurance Plan (UC SHIP) coverage will be canceled as of your status change (first day of the quarter), and your UC SHIP fee will be fully refunded. You are not eligible to continue your UC SHIP coverage past the date of your registration status change.

If you file your registration status change ON OR AFTER the first day of the quarter, your UC SHIP coverage will continue to the end of the quarter, and your UC SHIP fee will NOT be refunded. You will not be eligible to continue your UC SHIP coverage past the end of the quarter.

UC SHIP fee refunds will not be granted for retroactive status changes. This includes retroactive status changes based on “special circumstances” filed with the Office of the Registrar.

Note that the first day of the quarter is not the same date as the first day of classes. Refer to the Office of the Registrar's Academic Calendar for applicable dates.

SUMMER COVERAGE: UC SHIP members who withdraw or cancel registration during spring quarter or semester remain on UC SHIP and are covered through the end of the summer coverage period.

DATES OF COVERAGE: For dates of coverage, refer to UC SHIP 2011-12: Cost and Dates of Coverage.

Notification

If you change your registration status, you will NOT be individually notified of your loss of UC SHIP eligibility. Information regarding the impact of registration status change on UC SHIP eligibility is provided in the UC Davis Course Catalog, the Class Schedule and Registration Guide, and the SHCS insurance webpage. Anthem Blue Cross will provide a certificate to you upon cancellation of your membership, stating the beginning and ending dates of your UC SHIP coverage.
