UC SHIP Eligibility Notice: Dismissal
If you are dismissed, you may request a full refund of your UC SHIP fee and cancellation of coverage as of the effective date of dismissal (usually the first day of the quarter). Your UC SHIP Cancellation and Refund Request (pdf)must be submitted in a timely manner to Insurance Services at the UC Davis Student Health & Wellness Center, not to the Office of the Registrar or to Student Accounting. To request further information, call Insurance Services at 530.752.6055.
You are not eligible to continue UC SHIP coverage after the end of the term in which you are dismissed.
Notification
If you change your registration status, you will NOT be individually notified of your loss of UC SHIP eligibility. Information regarding the impact of registration status change on UC SHIP eligibility is provided in the UC Davis Course Catalog, the Class Schedule and Registration Guide, and the SHCS insurance webpage. Anthem Blue Cross will provide a certificate to you upon cancellation of your membership, stating the beginning and ending dates of your UC SHIP coverage.
