Hepatitis B Immunity Status: Admissions Requirement

The California Health and Safety code requires first time enrollees at the University of California who are 18 years of age or younger to provide a statement of immunization status for the Hepatitis B virus prior to enrollment.

Students who need to fulfill this requirement will need to fill out the online Hepatitis B Immunity Statement. In order to complete your immunity statement, you will need to document your full record of Hepatitis B virus vaccinations. This can be done in several different ways:

  • If you have received immunization for Hepatitis B (a series of 3 vaccination shots), you will need to provide the dates of each immunization along with the name, address, and phone number of the provider who administered the injections.
  • Students who have known immunity due to prior infection or possess an antibody titer showing immunity will need the date of illness/titer, and the name of the treating provider along with his/her address and phone number.
  • If your medical condition is such that receiving the Hepatitis B immunization series is not considered safe, please have your health care provider submit a written statement to:

  • If receiving this immunization is contrary to your beliefs, you have the opportunity to indicate this on the immunity statement.

New Students

Incoming students can access the Hepatitis B Immunity Statement through MyAdmissions (new students only).


Continuing Students

If you are a continuing student and are attempting to register for classes and you have a hold on your account, you may register for the current term but will be unable to add or drop classes until your immunity statement is complete. Use the following link to complete the Hepatitis B Immunity Statement (continuing students only) and select the appropriate information for your condition.
